Requirement Document
- 2 pcs 1X1 ID white background
- 2 pcs passport size ID with nametag, with white background and formal attire
- ( if married ) 3 Photocopies of Birth certificate or marriage certificate
- 3 Photocopies of TOR/FORM 137/ FORM 138
- ( Per Copy ) 6 Photocopies of 1 Valid ID with 3 original signatures
- 1 copy of Updated Resume
- 2 copies of Practicing COVID-19 Preventive Measures in the Workplace ( Vist LINK )
Eligibility Criteria
At least 18 years of age
Must have completed at least 10 yrs. basic education and ALS certificate of achievement with grade 10 equivalent holder
Must be a Filipino Citizen
