Note: There will be an entrance examination and an application fee of PHP 500.00

Requirements

• Interactive Recommendation Form (click here)
• PSA Birth Certificate
• 2×2 Picture with name tag and white background
• Certified True Copy of Grade 11 Report Card
• Certification of not enrolled to any Colleges or Universities*
• *for gap year students
